What it is

A Medicare model built around ongoing care.

ACCESS — Advancing Chronic Care with Effective, Scalable Solutions — is a voluntary model from CMS that supports people living with chronic conditions like high blood pressure, diabetes, and heart disease.

Who the model is for

People with Traditional Medicare

The model serves people with Traditional (Original) Medicare who have one or more chronic conditions. Medicare Advantage is not eligible. Final eligibility is determined by CMS and your care team.

We have been accepted as a participant in the ACCESS Model through IndyCare Carelink, our affiliated medical group. Whether and when it will be offered has not been determined. Sign up and we'll let you know if and when it becomes available.
