Physicians at IndyCare Carelink, the independent medical group affiliated with our platform, have been accepted into the CMS ACCESS Model in three clinical tracks: early cardio-kidney-metabolic conditions (hypertension, high cholesterol, obesity, prediabetes), cardio-kidney-metabolic conditions (diabetes, chronic kidney disease, cardiovascular disease), and musculoskeletal conditions (chronic musculoskeletal pain).
ACCESS is a new payment model from the Centers for Medicare & Medicaid Services. For independent pharmacies, it is another step toward the pharmacy becoming a true clinical access point for patients managing chronic conditions.
Acceptance is not the hard part. Delivering care consistently is. It takes software, documentation, training, and day-to-day operational support working together, with physician oversight behind every patient. Most independent pharmacies were not built to stand up all of that alone.
That is what Truentity was built for. We already run this model today through PACT, a physician-supervised, pharmacist-delivered care model for chronic disease. Physicians at IndyCare Carelink assess each patient and build the care plan. The pharmacy team delivers it day to day under that physician’s supervision, using the relationship it already has with the patient. In the cardio-kidney-metabolic condition areas, the model has produced 10 to 21 mmHg reductions in systolic blood pressure and 1.0 to 1.8 point reductions in A1C.
Pharmacies take part by partnering with Truentity. Your team delivers the day-to-day care under physician supervision through the affiliated medical group. We provide the platform, training, and operational support.
ACCESS won’t be the right fit for every pharmacy or every patient. We’ll review your patient panel with you and determine which program fits, whether that’s ACCESS, PACT, or both.
