What is the Rural Health Transformation Program?
A $50B, five-year rural health commitment.
RHTP is a $50B, five-year federal block-grant program. CMS sends funds directly to state applicants, who deploy capital across four categories: workforce recruitment and retention, technology modernization, preventive and chronic-disease programs, and service-line preservation.
NC's plan is being written — right now.
North Carolina is using its $213 million first-year RHTP funding primarily to establish locally governed NC ROOTS Hubs that coordinate medical, behavioral health, and social services, while also expanding prevention/chronic disease programs, behavioral health services, workforce development, value-based payment models, and digital health innovations across all rural counties.
